Adobe acrobat reader is enoughJust use adobe acrobat reader to type and print (if you have laser printer at office). You may need to add chinese character by hand though.

as to the Chinese characters, just print them out
and then cut and paste on the form, you can easily get good effect by photocopy the pasted form. it is easy to do.
